I am just getting the paperwork together for my visa, I have a couple of queries which hopefully someone can answer :

If you were adopted at birth do you have to put your original name down on the form as 'previous name'? (assume you do can someone confirm)

With regards 'full birth certificate' can you just provide the short certificate (no parent names) and your adoption papers and passport?

We have just submitted our parerwork and given full details and provided adoption certs and full birth cert. if you do not hold you can get the from GOV certs. They cost £25 each.

There are quite a few companies who do this just type in birth certificates in the search engine on google.

I was also adopted at birth. I know you cant use the short birth certificate. I telephoned births, deaths and marrages and they gave me the telephone number for the adopted birth certificates because they are at a different office. I just rang them up and they sent me a full certificate through the post, cant remember how much it was now, approx £15.00, I think. With the names you have to write on the form, I will have to ask my better half. I was adopted within the family, so my surname didnt change! I did however put my half brother on my Visa application but didnt put my birth mother!!! (my family is sooooooo, complicated, I didnt put her on because there was no more room left on the form!!! LOL! My friend also left her birth father off her visa form. I will check this with my better half when he gets home from work and let you know (he filled in all the paperwork!)
